STEP 1

Fill in a screen and press Review and send. Use student ID numbers only, never names or birthdates.

Your school's Microsoft Form opens with everything filled in. Press Submit and the entry is saved to your school's responses file.

Build the Report

Download the responses file from Forms and open it on the June report tab. Days and visits add up by month, and the newest record for each student counts.

Health Office Log

Enter one student visit at a time, or log a whole school day at once. Everything adds up by month on the June report.

Student Record

One record for each student with a diagnosed health condition. When anything changes, such as a new plan, a 504, or chronic absence, send the whole record again. The newest record for a student ID replaces the older ones.

Use the student ID number from your student information system. No names, birthdates or teacher names.

Diagnosed Conditions

Conditions diagnosed by a healthcare provider, at any time. Choose all that apply. Questions for each condition open under it.

Plans, Attendance and Medication Orders

Chronic Absence

Update just the chronic absence answer for students who already have a student record. Nothing else on their record changes. A student is chronically absent after missing 10% or more of school days, for any reason.

Screenings & Programs

Enter year-to-date totals, not just what's new. Each time you send this screen, it replaces the previous one, so always start from your latest numbers.

School Facts

Answers about the school as a whole. Fill this out in the fall and check it again in the spring. The newest version replaces older ones.

Nursing Staff

Everyone who provides school health services: RNs, LPNs and health aides. Credential counts and staff lists in the report come from this screen.

Import From Excel

Load a health tracking workbook and turn it into log entries. The import reads any tab with a Student ID column, a monthly table with month columns, question-and-answer tabs, and a staff list. It never reads names, birthdates or teacher names.
